Showing up late for an appointment may result in the cancellation of your appointment. If you are aware that you cannot make your appointment, please call ahead to reschedule. Please provide 24 hours’ notice if possible.

Do not bring anyone to your appointment. The only exception is if you are a minor, in this case bring your guardian.

Bring a valid picture ID. We are unable to accept expired identification.
